If our automatic check couldn't verify your identity during registration, you may confirm your identity and eligibility by submitting ID documents.

Please scan your original document in full color. Your document must clearly display your full name and address as you entered it at registration*

  • Driver’s License
  • Government-Issued ID Card
  • Voter’s Registration Card (including your address)

List C: Document to Confirm a Recent Name or Address Change (if different than documents in List A or B)

*If your name or address is different than what has is displayed on your other documents, please also provide a document from this list with your current information

  • Utility bill (no older than 2 months)
  • Marriage certificate
  • Divorce decree
  • Another document from List A which has not yet been provided

You may submit your documents in any of the following file formats: .doc, .docx, .jpeg, .png, .pdf, .rtf, or .jpg. Your scanned images must be clear and legible. Check your scanned images to make sure they're in color and aren't too dark, blurry, or otherwise unclear. Include images of both the front and back of each document.
